Gesundheit für Billstedt/Horn UG (limited liability)

We have been building an innovative, patient-oriented and cross-sectoral healthcare network in a deprived urban area since January 2017 together with partners. The long-term aim of this new model of care is to improve health-related chances of the population living in two neighbourhoods located in the East of Hamburg, Germany. The model pursues the triple aim: improving population health, improving patient experience and reducing per capita cost.

Lower health-related chances

Due to low socioeconomic status, people living in Hamburg’s neighbourhoods of Billstedt and Horn have lower health-related chances than the rest of Hamburg’s population. In these parts of the city there are more social welfare recipients, more people with a migrant background, lower levels of education and more single parents than in the rest of Hamburg. These citizens get ill earlier in life, are more likely to suffer from a chronic disease and have a lower life expectancy. At the same time, access to healthcare is limited – there are for example less primary care physicians and specialists in Billstedt and Horn than in other parts of Hamburg. This may be one reason why the use of emergency care services has increased over the past years.

An new model of population-based and cross-sectoral care

The chance to live a healthier life in Billstedt and Horn will be improved. In order to achieve that, the project partners will implement a patient oriented and interdisciplinary health network. It aims to:

Project duration and funding

The innovation fund of the Federal Joint Committee (Gemeinsamer Bundesausschuss, G-BA) financially supported the project for three years with up to 6.3 million euros (start date: 1st of January 2017). Now it will continue to be funded: The relevant contracts were officially signed by the four participating health insurance companies (AOK Rheinland/Hamburg, Techniker, BARMER and DAK GESUNDHEIT) in December 2019.
